Skilled Nursing

  • Assess for changes in medical condition

  • Education regarding disease management

  • Teach how to manage and administer medication

  • Perform medical procedures such as catheter insertion, wound care, ostomy care, lab work and injections

Occupational Therapy

  • Focus on the ability to perform activities of daily living such as bathing dressing, and household tasks

  • Evaluate the need for and train in the use of adaptive equipment to aid in performing activities of daily living

  • Teach energy conservation techniques

  • Instruct in home exercise program

Physical Therapy

  • Focus on exercises and activities to improve balance, strength, and endurance, to reduce the risk of falls

  • Improve ability to ambulate and/or transfer

  • Evaluate the need for and train in the use of devices such as walker, canes, wheelchairs

  • Instruct in home exercise program

Home Health Aide

  • Provided when you are receiving skilled nursing, physical, occupational, or speech therapy

  • Help with personal care on a short-term basis, such as bathing. dressing, toileting, light meal preparation
